Auto-Chart automatically creates a new patient record when no existing chart is found and confidence rules are met.
This is most useful for new-patient referrals or when your EMR frequently receives first-time patient documents.
Important: Auto-Chart is intended for non-referral workflows and works in tandem with Auto-Save. For referral workflows, use Auto-Referral, which creates the referral and new chart as needed.
How it works
- The document is recognized as a type allowed for chart creation (often “Referral”).
- The system searches for a matching patient.
- If none is found, it checks confidence for patient name and other identifiers.
- If confidence meets your threshold, a new chart is created in your EMR.
- If Auto-Save is also active, the document can then be filed automatically into the newly created patient chart.
Key parameters
- Min Confidence for Document Classification – Required certainty that this is the right document type before Phelix creates a new patient chart.
- Confidence for Patient Name – How closely the document’s patient name must match to create a record.
- Confidence for Patient Identifier – Optional: confidence based on ID or other demographic match.
Example
A clinic receives a non-referral document for a patient who is not yet in the EMR.
The system recognizes the document type with high confidence, extracts a clear patient name and date of birth, and meets your patient confidence threshold.
Auto-Chart creates the new patient chart and, if Auto-Save is enabled, the document can then be filed automatically into that chart.
Common reasons Auto-Chart holds back
- Missing or unclear patient demographics
- Document type not permitted for chart creation
- Confidence below threshold for name or identifier match
In these cases, the fax stays in the worklist for staff review — no data loss occurs.
